What if I want the Office of Student Conduct to disclose my conduct record to another person, institution, or agency or to obtain a copy for myself?

You will need the following form(s):

  • Student Consent for Release of Disciplinary Records
What if I want to have someone serve as an Advisor (the person may or may not be an attorney)?

You will need the following form(s):

  • Notice of Support Participation*
  • Advisor Certification
What if I want to have someone serve as a Support Individual (the person may or may not be an attorney)?

You will need the following form(s):

  • Notice of Support Participation*
What is the best way to submit a completed form to the Office?

There is no one best way - it will depend upon what is easiest for the student. Forms may be submitted using any of the following methods:

  • Hand-deliver it to the Office (324 Plemmons Student Union)
  • Email it to the Office (studentconduct@appstate.edu)
  • Fax it to the Office at (828)262-4997
  • Mail it to the Office (Room 324 Plemmons Student Union, P.O. Box 32084, 263 Locust St., Boone NC 28608)
What is the best way to complete the Academic Integrity Process (for faculty)?
  • If the student meets the requirements to move forward with an informal resolution, the step-by-step process for an informal resolution can be found on our website.  
  • Once completed, the form will need to be sent to our office.  Send original to: Office Of Student Conduct via the "Report Academic Integrity Incident" form on our main page.
