Which form needs to be completed if I want the Office of Student Conduct to disclose my conduct record to another person, institution, or agency or to obtain a copy for myself?
Answer: Only the Student Consent for Release of Disciplinary Records form.
Which forms need to be completed if I want to have someone serve as an Advocate (the person may or may not be an attorney)?
Answer: Both the Notice of Support Participation form* and the Advocate Certification form.
Which form needs to be completed if I want to have someone serve as a Support Individual (the person may or may not be an attorney)?
Answer: Only the Notice of Support Participation form*.
What is the best way to submit a completed form to the Office?
Answer: There is no one best way. It will depend on what is easiest for the student. The student has the
